  • Johnathan Swofford FNP-C, CPR, ACLS, & PALS

    FAMILY NURSE PRACTIONER

    Johnathan Swofford NP is currently serving as the nurse practitioner at Array Primary Care and brings expertise in primary care.

    Johnathan graduated with a 4.0 from Walden University’s Nurse Practitioner Program and holds a Bachelors of Science in Nursing from The University of Texas at Arlington. His commitment to continuous learning is evident through certifications such as FNP-C, CPR, ACLS, and PALS.

    Beyond the medical field, Johnathan finds joy in travel, quality time with family and the calming pursuit of fishing.

    His multifaceted career and dedication to both clinical excellence and community service exemplify a holistic approach to healthcare.

    Johnathan’s journey includes roles as a 360 Department Nurse Practitioner and in the Transitional Care Clinic.

    Johnathan’s early career involved pivotal roles as a Navy Corpsman with Marine Corps units also worked as Flight Nurse and Paramedic at CareFlite and as a Firefighter/paramedic with the Flower Mound Fire Department. He has also contributed significantly as the EMS Director at Parker County ESD #1.

  • Heather Smith, MSN, APRN, FNP-C

    FAMILY NURSE PRACTITIONER

    Heather is a Family Nurse Practitioner with a passion for providing compassionate, personalized care to patients and their families. Before becoming a nurse practitioner, she spent 14 years as a registered nurse, with a background in cardiac and critical care nursing. Her nursing career has included caring for patients in both Texas and Arizona, giving her a broad clinical perspective and a strong foundation in patient-centered care.

    Heather is a proud graduate of Northwest High School and earned both her Bachelor of Science in Nursing and Master of Science in Nursing from Texas Woman’s University.

    Family, faith, and service are important parts of Heather’s life. Coming from a family with a strong military heritage, she has a deep appreciation for the military and the sacrifices made in service to our country. Outside of the clinic, she enjoys spending time with her husband and son, especially making memories at the lake. She also loves traveling, gardening, camping, and enjoying time with family.

    As a Family Nurse Practitioner, Heather’s goal is to build lasting relationships with her patients and encourage each person to be the healthiest version of themselves. She believes in continuing to learn throughout her career and is committed to growing in her role as a primary care provider. Most importantly, she hopes to provide care that makes patients feel heard, respected, and supported throughout every stage of life.

  • Charles Tibbles MD

    FAMILY DOCTOR

    Dr. C. Kelley Tibbels MD started private practice of Family Medicine in 1981, joining Dr. Ben Merritt and Dr. Tom Long in clinical and hospital practice including obstetrics. He earned his MD at the University of Texas Medical School at Houston. He came to know Wise County while working in the emergency room on weekends at Decatur Community Hospital beginning in 1976 while he completed his military obligation and internship and residency in Family Medicine at Carswell

    AFB in the United Stated Air Force. His career has evolved to consulting and Medical Director and private practice at Array Primary Care and Faith Community Clinic at Alvord. He is also an active Medical Director at Solaris Hospice and nursing homes in Decatur. His community contributions to Wise County include previous memberships in local service organizations, serving on the Decatur Independent School District Board, Hospital Chief of Staff. and was Decatur's Citizen of the year. He enjoys family, marriage to his lovely wife Sandy for 54 years, 4 children, 4 grandchildren and 2 great grandchildren. Music is a passion, occasional woodworking, tinkering with technology and reading are other avocations. His life is devoted to spreading the Grace and Love of Jesus Christ.

  • Mauro Ganzon

    My name is Mauro Ganzon, and I have practiced in the Wise County area since 2003. I graduated from college at the University of Texas at Austin and attended medical school at the University of Texas at Southwestern before doing internship and residency at John Peter Smith Hospital in Fort Worth.

    I truly believe that practicing family medicine is a calling and have enjoyed seeing complete family units - a few up to four generations.

    I have enjoyed being a small town doctor and feel blessed to have the opportunity to be involved in the care of my patients.
